O Governo do Distrito Federal, por meio da Portaria nº 104, de 24 de maio de 2012, aprovou os modelos de formulários para acesso à informação pública, no âmbito do Poder Executivo Distrital. Os formulários para solicitação de informações se aplicam tanto à Pessoa Física como à Pessoa Jurídica, que queiram obter informações produzidas ou custodiadas pelos órgãos e entidades do Poder Executivo local. Tal medida servirá para padronizar todos os pedidos de acesso feitos aos diferentes Órgãos da Administração Direta e Indireta do DF.

Os formulários a que se refere essa Portaria serão disponibilizados no sítio institucional dos órgãos e entidades do Poder Executivo do GDF e nas unidades do Serviço de Informação ao Cidadão - SIC.
